Delivery chain

The online delivery chain is a self assessment tool to support commissioners in the planning and implementation of polysystems within a primary care setting.

The tool has been developed to aid commissioners in assessing their approach to delivering polysystems, or other service transformation initiatives in primary and community care, and to support the development of robust planning across all critical components of delivery.
