Care packages
As part of our operating plan for 2010/11, we have worked with the North Central London sector to support their care pathway development. Originally designed to cover several sector-wide care pathways, our work assisted the sector commissioners and clinicians to create a means of presenting financial and commissioning assumptions, including service redesign and costs, for validation.
The work was undertaken on the basis of an agreement that it could be shared with other sectors and PCTs in London. The purpose is to enable commissioners access to a pre-populated finance and commissioning tool when redesigning care pathways and commissioning care packages. The work has also contributed to the NHS London work undertaken to benchmark costs for primary and community services across London.
