Good Practice Guide

The End of Life Care Good Practice Guide and its accompanying six quality markers aims to support London’s providers and commissioners improve end of life care in the capital.

It advises London-based commissioners and clinicians on how to implement a model of care that provides for at least 50% of deaths outside of hospital by March 2013 and to improve the experience of all patients who die while being cared for by health and social care services.

The guide proposes a number of quality markers and standards that PCTs are encouraged to incorporate into their commissioning plans, together with effective mechanisms for monitoring the processes and outcomes.
