Transition Support Programme

This programme ran from 2009-2011 to raise the standards of transition to adult life in all local areas.

Children and families programme advisors both supported and appropriately challenged London authorities who had been assessed by the national Transition Support Programme as meeting statutory responsibilities, but requiring further work to develop and improve outcomes for young people (Development Stage 2).

Following the closure of the both the Transition Support Programme and the Children and Families Programme, a new website with transition information has been launched.
