Care planning and self management
Diffusing care planning and encouraging structured self management across London is a fundamental element of the diabetes model of care. This project is working with five sites across London to support the identification and subsequent commissioning of a locally appropriate approach to care planning and self management.
Learning from these sites will inform commissioning guidance for care planning which will be embedded in current commissioning systems from March 2011.
This work will also conduct a review on enabling structures that support effective implementation of care planning – for example local diabetes networks. This project supports the focus of health policy for putting the patient at the centre of their care.
